Benefits Summary:

Blue Cross / Blue Card PPO

The PPO Plan is a comprehensive medical plan that covers a major portion of your medical expenses.  You have the freedom to go to any physician or health facility, but utilizing certain “preferred providers” minimizes your out-of-pocket costs.  If you go to a network provider, you generally pay $10 per visit with most other services covered at 90%, after the deductible.  Visiting a non-network provider is covered at 70% after satisfying the calendar year deductible.  The plan you choose during your eligibility period is the plan you will have until the next open enrollment period. Opsware’s open enrollment is in November of each year with open enrollment changes effective January 1st.

HMO Plans

Depending on your location, you may be eligible to participate in one of our HMO’s:  Blue Cross HMO, Kaiser (California only).  These plans generally provide a broad spectrum of medical services through a network of physicians and hospitals. You will need to choose a primary care physician for you and each of your dependents. Most services are covered at 100% after a $10 co-payment.  The plan you choose during your eligibility period is the plan you will have until the next open enrollment period. Opsware’s open enrollment is in November of each year with open enrollment changes effective January 1st.

The Delta Dental Plan

Opsware also offers a dental plan through Delta Dental.  The Plan provides for preventive, diagnostic, and restorative services.  The greatest benefit is provided if you receive treatment through the network of participating dentists.  If you go to a dentist outside the network you are still eligible for services, however, your out-of-pocket cost may be higher.  

Vision Coverage—Vision Service Plan (VSP)

You and your eligible dependents are automatically enrolled in the Vision Plan when you enroll in the dental plan.  The vision plan through VSP provides one eye examination and lenses each 12 months and frames each 24 months.  The greatest benefit is provided if you receive treatment through the network of VSP doctors. 

Basic Life and Accidental Death and Dismemberment (AD&D) – Aetna

Opsware provides employees with life and accidental death and dismemberment insurance equal to two times your annual salary, up to a maximum of $500,000. 

Income Replacement—Short Term and Long Term Disability— Unum/Provident

Short Term Disability – If you are unable to work due to an illness, injury or accident for more than 7 consecutive days, Opsware will provide you with short-term disability in the amount of 75% of your salary.  If you live in a state where State Disability is offered, your short-term disability will be coordinated with any State Disability earnings. 
Long Term Disability – In order to ensure some financial protection for you and your family in the event of a disability, Opsware provides long-term disability coverage for all employees.  Once you have been disabled for 90 days, the Plan provides benefits in the amount of 66 2/3% of your salary up to $10,000 per month.  This benefit is coordinated with any other disability payment for which you may be eligible. 

Section 125 Flexible Benefits – Creative Benefits

  • Health Care Expense Account – The Health Care Expense Account allows you to use pre-tax dollars to pay for expenses not 100% covered under your health plan.
  • Dependent Care Expense Account – The Dependent Care Expense Account allows you to use pre-tax dollars to pay for eligible day care and elder care which enables you and your spouse, if applicable, to work

Employee Assistance Program (EAP)—Concern

Sometimes it can be difficult to coordinate all the different aspects of your life.  Things can happen to disrupt even the best plans.  The CONCERN EAP offers employees and their eligible dependents private counseling and referrals to resources and agencies through the CONCERN network.  This program is provided to you at no cost and is completely confidential.

Business Travel Accident (BTA) – AIG 

Employees required to travel on behalf of the company will be covered under our Business Travel Accident Plan.  The BTA plan provides a Life Insurance benefit, travel assistance benefits and medical evacuation and assistance benefits.  This benefit is provided at no additional cost to you.

401(k) Retirement Plan

The Opsware 401(k) Plan allows you to invest today for your retirement tomorrow.  You can contribute up to 1-60% of your salary on a pre-tax basis.  This money is then invested in fund options of your choice—you have a variety of funds to choose from through Fidelity.  The plan is designed to accommodate the conservative all the way to the more aggressive investor. 

Employee Stock Purchase Program (ESPP)

The Employee Stock Purchase Program provides a way for Opsware employees to purchase Opsware stock at a 15% discount of the fair market price through payroll deductions.  You may contribute from 0% to 15% of your salary on an after tax basis.  Information about the ESPP program will be communicated before each new purchase cycle.

Vacation

Employees earn 15 days of vacation (accrued on a monthly basis) each year during their first five years of employment.  After the sixth year, employees earn one additional day of vacation each year up to a maximum of 20 days.

Holidays

In observance of traditional times of recreation and celebration, Opsware has designated 10 paid.  The scheduled holidays include New Year’s Day, Martin Luther King Day, President’s Day, Memorial Day, Independence Day, Labor Day, Thanksgiving Day and the day following, and Christmas.
